IMPORTANT NOTICE - After reading the COTS  'Introductory to Surrogacy'  (cots booklet available on the downloads page) you wish to proceed, you will need complete the Details of Couple Form (you will find a downloadable version of this further down this page) and forward this to COTS with a cheque for £850.

The form is available to download and print or you can fill it in yourself and send to us with your fee. Please note that
the joining fee is non-refundable unless COTS does not accept your application.  Also complete the Contract.

Once your membership has been processed a support worker will contact you to arrange an Information Session. This is not an assessment but an opportunity for you to find out more about every aspect of surrogacy. The meeting must take place in your own home and usually lasts about two hours.

Couples living outside the UK - We are sorry but COTS cannot accept people who do not live in the UK.

CRITERIA FOR JOINING OUR ACTIVE LIST:
  • Intended parents must have exhausted ALL other medical channels in trying and have a child of their own, unless their doctor has indicated that pregnancy could be harmful to the intended mother .
  • We require both intended parents to be checked by the Criminal Records Bureau, the support worker will complete the necessary forms when they come to see you.  The costs of this is included in the initial subscriptions.
  • We need copies of all Medicals. If these have not already been carried out the support worker has the necessary kits. The costs of having a surrogate tested is included in the initial subscriptions, but intended parents will have to pay for their tests direct to the laboratory.
  • COTS will NOT accept any intended parents who are NOT prepared to inform their child and close family members of his or her origin.
